Szczegółowy opis przedmiotu zamówienia znajduje w Formularzu asortymentowo-cenowym, który stanowi załącznik do SWZ. Wykonawca składa ofertę na wybraną CZEŚĆ lub CZEŚCI wypełniając odpowiednio Formularz asortymentowo-cenowy (dane należy wpisać czcionką o rozmiarze co najmniej 10). Części dla których Wykonawca nie składa oferty należy usunąć z formularza.
